II. Your Baptismal Ministry
Answer questions on separate sheet and attach to application form.

1. How long have you been a member of your current congregation?

2. How are you involved in your congregation at present (e.g., do you hold an elected or appointed office, what committees or task forces do you belong to, what activities are you engaged in)?

3. How are you involved in activities at the Diocesan level (e.g., member of a task force or commission, volunteer for a program or mission which the Diocese sponsors) or in the church at large?

4. Read through pages 304-305 of the Book of Common Prayer. How do you relate the roles/activities in which you are engaged with your congregation or the Diocese to your baptismal covenant?

 

III. How You Would Use the Grant
Answer questions on separate sheet and attach to application form.

1. For what purpose do you seek a Lay Ministry Grant (be specific)? If you have received a Lay Ministry Grant in the past, please state how this application relates to your previous grant.

2. How does this relate to your current or future ministry within your congregation, the Diocese or the church at large? Please be specific (e.g., the training will better prepare me to teach Sunday School, conference will provide opportunities for me to learn new stewardship techniques, will be starting a new program of counseling for immigrants in our neighborhood). If the grant will serve as seed money or provide start-up for a new project, please give a timeframe for launching the new effort and indicate how the project will be sustained after the grant period has ended.

3. How large a grant are you requesting?

Will this amount cover the entire costs of your project/program?

Please attach a budget showing the total cost of the project or program, the amount you are requesting under this grant program, and noting any additional sources of financial assistance with this project. (We strongly encourage financial support from your congregation and would like to see that noted in your budget, if available.)
